Request Public Records via FOIA

  1. Please Note: Once you submit a FOIA request, your name and information about the request will be made available on the City's website, as it will become public information.

  2. It is a violation of the Freedom of Information Act for a person to knowingly obtain a public record for a commercial purpose without disclosing that it is for a commercial purpose, if requested to do so by the public body. 5 ILCS 140.3.1(c).

  3. If you are requesting that the public body waive any fees for copying the documents, you must submit a statement of the purpose of the request, and whether the principal purpose of the request is to access or disseminate information regarding the health, safety and welfare, or legal rights of the general public. 5 ILCS 140/6(c).

  4. Please describe the public records you wish to recieve. In order to expediate the search for the records, please be as specific as possible.

  5. The City will respond to this request withing five (5) working days.

  6. By clicking "I agree," you agree and acknowledge that 1) your application will not be "Signed" in the sense of a traditional paper document and 2) By signing in this alternate manner, you agree that your "electronic signature" is valid and binding upon you to the same force and effect as a handwritten signature.
